Driving Licence Name Change Form 2 in Karnataka: RTO Process

What Is Form 2?

Form 2 is the Parivahan application used to change or update the particulars on a driving licence, including your name. It is the standard form your RTO uses when you request a name change on an existing licence.

You can get the form from the Parivahan portal or at your RTO. It records your licence number, your old name, your new name, and the reason for the change, and it is submitted along with your supporting documents.

One Form, Six Jobs: Tick the Right Box

Form 2 is not a name change form. It is one form that does several jobs, and your name change is one of them. Reading its own title tells you most of what you need to know.

• Its official title lists everything it covers — an application for a learner's licence, or a driving licence, or the addition of a new class of vehicle, or renewal of a driving licence, or a change of address or name.
• So there is a service selection near the top — the form asks you to tick against the service you are applying for.
• You can tick more than one — the form expressly allows a single or multiple service to be selected where applicable.
• Which is a genuine saving — a name change and an address change on the same form usually means one visit and one set of verification rather than two.
• Tick nothing and the file has no purpose — an unticked service selection is one of the quieter reasons a form is handed back at the counter.
• It is prescribed under the rules — Form 2 sits under the Central Motor Vehicles Rules, 1989, which is why the same form is used across the country rather than varying by state.

How to Fill the Form

• Licence number — enter it exactly as on your current licence.
• Old name — as printed on your licence today.
• New name — exactly as on your proof and Aadhaar.
• Reason for change — marriage, or a legal name change.
• Address and contact — your current details.
• Signature — as required by your RTO.

Form 1 and Form 1A: When You Need One

These are separate forms from Form 2, and turning up without the one that applies to you is a common reason a file is not accepted. Neither of them is a name change form.

• Form 1 is a self declaration of physical fitness — a declaration you make yourself, used for a licence to drive a vehicle other than a transport vehicle.
• Form 1A is a medical certificate — issued by a registered medical practitioner, and used where a transport vehicle licence is involved.
• Form 2 sets out when Form 1A is expected — where the applicant is applying for renewal and is above forty years of age, or is applying for a transport licence.
• Form 1A does not last — a medical certificate issued in Form 1A is valid for one year from the date it is issued, and it is accompanied by a passport size photograph.
• For a plain name change, neither is usually the issue — but if you are combining the name change with a renewal and you are over forty, plan for the medical certificate.
• So Form 1 is not "the name form" — several guides say it is, and following that advice means arriving with the wrong paper.

The Declaration You Are Signing

The bottom of Form 2 carries a declaration. It is short, people sign it without reading, and it is worth understanding what you are stating.

• You declare the facts and documents are true and genuine — to the best of your knowledge and belief, which covers everything you have written and attached.
• You state your disqualification status — the form asks you to declare whether or not you have been disqualified from holding a driving licence.
• Which is why the name has to be exact — a name written approximately is not a small matter on a document you are certifying as genuine.
• The form also lists what proof it will accept — including an affidavit sworn before an Executive Magistrate, a Notary Public or a First Class Judicial Magistrate, among the acceptable proofs of age.
• Where one document proves two things — the form notes that you select only one where the same proof serves for both address and age, so you are not asked to duplicate.
• A lost or damaged licence has its own entry — a copy of the police complaint is listed where the licence was lost, mutilated, defaced or damaged.
