The affidavit is a short legal statement made on non-judicial stamp paper and sworn before a Notary Public. It records three things: your old name, your new name, and the reason for the change, which for you is marriage. You need one only on the Gazette route. If you are simply taking your husband’s surname with a photo-bearing marriage certificate, no counter listed above will ask you for an affidavit.
