Do not have a certificate yet?

Register the marriage. In Karnataka this is done through the Kaveri portal or at the Sub-Registrar’s office, and it falls under the Sakala service guarantee, so the certificate is typically issued quickly rather than in weeks. The affidavit is a short legal statement made on non-judicial stamp paper and sworn before a Notary Public. It records three things: your old name, your new name, and the reason for the change, which for you is marriage. You need one only on the Gazette route. If you are simply taking your husband’s surname with a photo-bearing marriage certificate, no counter listed above will ask you for an affidavit.

  2. A clipped newspaper advertisement without the masthead or date visible. Keep the full page.
  3. The notice supplied as a PDF or photograph rather than an editable Word file. The office typesets from it.
  4. Angled or shadowed phone scans where a corner is cut off or the text is not readable at full size.
  5. PAN and Aadhaar already disagreeing before the marriage change is even filed.
  6. Unsigned photocopies. Self-attest each one with the date.
  7. An affidavit signed but not sworn before a Notary Public. A signature alone is not a notarisation.
  8. The BharatKosh receipt in someone else’s name. The Government challan should be generated in yours.

My name is already spelled differently on Aadhaar and PAN. What now?
Fix that mismatch before filing the marriage change. Doing both at once usually gets the file returned. A PAN that does not reconcile can be treated as inoperative, which raises tax deducted at source under section 206AA.
The name on my marriage certificate is misspelled. What do I do?
Correct the certificate at the registering authority first, before you touch any other record. Everything downstream copies from it, so an error there multiplies through every application you make.
I married outside Karnataka but live here now. Does that change anything?
No. A certificate issued in another State is valid. Your address proof should show your current Karnataka address, which is what the local offices will check against.
Does my husband need to sign anything?
Only Annexure J and some bank forms need his signature. The affidavit and the Gazette application do not. His photo ID is worth carrying, because banks and the passport office often ask to see it.
What if I lose the newspaper cuttings?
Ask the newspaper for a certified tear-sheet. It is usually possible and always cheaper than paying for a fresh advertisement, which runs Rs 500 to Rs 1,500 and costs you another three to six weeks.
What is the most common reason a file is rejected?
The same name spelled two different ways across your own documents. Write your maiden name and married name on one sheet in the exact spelling you want, and copy from that sheet every single time.
Can I revert to my maiden name later?
Yes. Reverting is a name change in its own right and usually needs the Gazette route, because a marriage certificate proves the change into your married name, not out of it.
Is an address proof in my maiden name a problem?
No, and it is expected. The entire reason you are applying is that your records are still in the old name. No counter treats that as an inconsistency.
How long is this all going to take?
On the short list, about two to six weeks across every record, with the passport the slowest. On the Gazette route, add three to eight weeks for publication before you begin the record updates.
