A passport is a national document, so its name cannot simply be edited. When a child's name changes, the parents apply for a passport re-issue and the passport office prints a fresh passport with the new name. The old passport is surrendered during the process.

The Passport Fee Schedule Was Replaced This Year

Every page quoting passport fees for a child is now quoting an old schedule. The Passports (Amendment) Rules, 2026 were notified on 25 June 2026 and took effect from 1 July 2026, replacing Schedule IV of the Passports Rules, 1980 with an updated schedule of fees.

• A concession was introduced — ten per cent off for children up to the age of eight, and for applicants over sixty.
• It does not apply to you — the concession is for fresh applications made within India. A name change is a re-issue, so the concession does not reach it.
• The amounts moved — because the whole schedule was substituted, any figure published before July 2026 should be treated as out of date, including figures repeated confidently elsewhere.

A Minor's Passport Ends at Eighteen, Whatever Its Date

A passport issued to a minor is valid for five years or until the holder turns eighteen, whichever comes first. That single rule changes how you should time a name change, and no other page on this subject mentions it.

Who Must Attend, and the Annexure Nobody Agrees On

Search this question and you will be told, with equal confidence, that you need Annexure C, D, G or H. They cannot all be right. The lettering has been reorganised more than once, including a round of changes that cut the number of annexures and made the survivors plain-paper self-declarations.

• The substance is stable, the letters are not — what is always needed is a declaration by the parents about the minor, and a route for the situation where only one parent can be there.
• Both parents attending with the child is the position — Passport Seva has directed that both parents visit the Kendra with the minor applicant, submitting the parental declaration with their own passports where they hold them.
• Where only one parent can attend — there is a specific declaration for that, submitted with that parent present at the Kendra.
• Where a parent is abroad — that parent's declaration can be attested by the Indian Mission or Post abroad rather than requiring them to fly back.
• On plain paper — the parental declaration for a minor is given on plain paper. Do not pay a notary for something the form does not ask to be notarised.
• Take the letter from the source — use the annexure letter shown by Passport Seva's own document advisor for your application type on the day you apply, not from any article, this one included.

For Children Born From October 2023, the Birth Certificate Is the Proof

A separate change matters if your child is a toddler. Following the amendment to the birth registration law that took effect on 1 October 2023, the passport rules were amended so that an applicant born on or after that date must produce the birth certificate as proof of date of birth.

• Born on or after 1 October 2023 — the birth certificate issued by the Registrar or municipal authority is the proof of date of birth for the passport.
• Born before that date — the older alternatives remain available, including a school leaving certificate, PAN, driving licence or voter ID.

• And it must match the gazette — the certificate, the gazette and the application should show the same name, spelled the same way.

Fix the Supporting Records Before the Appointment, Not After

The passport is the last record to change, not the first. Everything the counter checks it against should already agree with the gazette by the time you arrive.
