A passport is a national document, so its name cannot simply be edited. When a child's name changes, the parents apply for a passport re-issue and the passport office prints a fresh passport with the new name. The old passport is surrendered during the process.
The passport re-issue application asks for documentary proof of the new name. For a child, a gazette notification is the cleanest and most widely accepted proof. Doing the gazette first means the passport application goes through smoothly, without back-and-forth queries.

Publish the child's new name in the gazette first, through the affidavit and newspaper route. This is the proof the passport office needs.
On the Passport Seva portal, choose re-issue and select "change in existing personal particulars" for the name change.
Book a slot at the Passport Seva Kendra or Post Office Passport Seva Kendra and pay the re-issue fee.
Both parents attend with the child, carrying the gazette, birth certificate, current passport, and parents' documents.
After verification, the new passport is printed with the child's updated name and dispatched.
Both the State and Central Gazette are valid, but for a passport the Central Gazette is the safer choice, since the passport is issued by the Government of India.

Every page quoting passport fees for a child is now quoting an old schedule. The Passports (Amendment) Rules, 2026 were notified on 25 June 2026 and took effect from 1 July 2026, replacing Schedule IV of the Passports Rules, 1980 with an updated schedule of fees.
A passport issued to a minor is valid for five years or until the holder turns eighteen, whichever comes first. That single rule changes how you should time a name change, and no other page on this subject mentions it.

Search this question and you will be told, with equal confidence, that you need Annexure C, D, G or H. They cannot all be right. The lettering has been reorganised more than once, including a round of changes that cut the number of annexures and made the survivors plain-paper self-declarations.
A separate change matters if your child is a toddler. Following the amendment to the birth registration law that took effect on 1 October 2023, the passport rules were amended so that an applicant born on or after that date must produce the birth certificate as proof of date of birth.
The passport is the last record to change, not the first. Everything the counter checks it against should already agree with the gazette by the time you arrive.
